This session will review wound assessment and wound care interventions that meet practice standards and regulatory guidelines. In addition, the content will explore why some wounds may never heal or continuously open and close, are not able to sustain healing, and how to document this issue. Lastly, information on end-of-life (EOL) wounds, such as the Kennedy Terminal Lesion/Ulcer, skin failure and other wounds that appear as EOL approaches will be reviewed with current guidance from CMS on documentation and reporting.
